• Adults requiring specialist Dementia care

• Adults with Alzheimer’s, Parkinson’s disease or multiple sclerosis

• Adults requiring palliative and end of life care
• Those requiring high dependency care – two carers attending
• Adults with auditory, visual or speech impairment
• Adults with physical disabilities
• Adults with some learning difficulties
• Adults experiencing some mental health problems 
• Older people in supported living schemes
• Those returning from hospital
• Those requiring convalescence, respite or day-care support
• Those requiring a sit-in service
• Those requiring a transportation service

(Adult refers to those over the age of 18)

The services we provide:

Our flexible services are completely bespoke and tailored to suit you and your family.  Visits can vary from a minimum of 30 minutes to one or more hours, one or more times per day from one to seven days per week, 365 days per year.  We also offer a return from hospital (reablement) package to help people regain their independence and confidence around the home as well as complete palliative and end of life care packages. An assessment of individual need will identify the services and the frequency required but our personalised comprehensive range of care covers assistance with daily activities and a wide range of auxiliary services:

• General Personal Care

• Medication support
• Meal preparation

• Practical tasks and light domestic including assisting with pet care
• Supplies shopping
• Social and companionship visits
• Appointment attendance
• Companionship outings including afternoon teas, meals out, theatre trips, accompanied shopping and the like
• Sitting service/support to partners or main Carers
  

In addition, we have recently introduced a ‘Home Again’ service – a 4 hour package to collect you from hospital, support you home and settle you in safely.  This package will generally make sure you are warm and comfortable ensuring that your immediate needs are covered until friends or family can take over. This service can include shopping and meal preparation.
